Required Documents

The required documents for the Laboratory Registration include:

  • Base Building Hazardous Materials Report (provided by a qualified person, firm, or corporation)
  • Laboratory Compliance Report (provided by an approved FPE)
  • Hazardous Material Inventory Statement (HMIS)
  • Hazardous Material Inventory Summary/MAQ Analysis (one for each control area and or laboratory suite)
  • The .t2s file
  • Emergency Action Plan
  • Chemical Hygiene Plan
  • Laboratory Floor Plans that identify:
    • Laboratory Suite and Control Area boundaries
    • Non-Lab Space
    • Chemical Fume Hoods
    • Biosafety Cabinets
    • Flammable Storage Cabinets
    • Hazardous Material Storage Cabinets
    • Cryogenic Storage or Use
    • Compressed Gas Storage or Use
    • Chemical Storage Room (if applicable)
    • Emergency Wash Stations (safety shower and or eye wash station)
    • Fire Extinguishers
  • Evacuation Floor Plans identifying primary and secondary evacuation routes and rally point.

(Please note that Chemical Fume Hoods and Biosafety Cabinets must have a unique I.D. number associated with it on the floor plan. The floor plans no longer need to be sized to 8.5” X 11”. Please use a digital format with sufficient resolution)
